Clinical Performance Instrument
- The assessment tool utilized during all clinical education experiences is the APTA's Web Clinical Performance Instrument. In addition to the clinical instructor's assessment, students will complete a self-assessment at mid-term and final. Completion of a 90 minute online training module is required before initial access to the instrument will be allowed. For more information, please refer to the APTA instructions.
Physical Therapist Student Evaluation: Clinical Experience and Clinical Instruction Form
- Students complete the Physical Therapist Student Evaluation: Clinical Experience and Clinical Instruction form from the APTA to provide feedback relative to the quality of the site and the clinical instruction received at each site. Sites, Site Coordinators of Clinical Education, and Clinical Instructors are welcome to review the documents to be made aware of the areas in which a site and CI are being assessed prior to the student coming on site. Please do not hesitate to reach out to the Clinical Education team with any questions about the forms.
