Forms for thesis/dissertation, programs, certificates and graduation
- Thesis/Dissertation Committee Appointment
Indicates the individuals that will serve on a student's degree advisory committee. Check with your graduate program coordinator to determine if this form is required. Required of all students doing a thesis or dissertation project. Submit by the end of the first semester of attendance (if applicable to the degree)
Theses and Dissertation Standards workshops will be hosted in the Fall and Spring semesters (specific dates will be announced by the Graduate School).
- Course Substitutions
Graduate course substitution petitions are entered by program directors/advisors via Dynamic Forms. Students seeking to have a specific course count toward their program of study must consult their program director/advisor, who will determine whether the course is an acceptable substitute for the program’s requirements.
Timely submission of substitution requests is required to ensure accurate reflection in a student’s MyMocsDegree and to avoid graduation delays. All course substitutions require final approval from the Graduate School Dean (or designee)- Graduate Degree Examination Results
Used to record the results of comprehensive, preliminary and final defense examinations.
Forms for grades, petitions and individual studies contracts
- Individual Studies Contract
Registrar Office form to be completed for individual studies courses. Submit when registering for the course. This form must be submitted when registering for any individualized course.
- Grade Appeal
If a student believes that their rights and interests have been seriously jeopardized by an unfair, arbitrary, or malicious exercise of faculty grading authority, they may appeal the grade (see graduate catalog for full policy, as well as grade appeal procedure and timeline). Failure to receive the grade desired or expected is not sufficient reason to appeal a grade. The Grade Appeal Form should be utilized for such appeals.
- Backdated Withdrawal
To request withdrawal from a course after the official withdrawal date. Backdated withdrawals are only considered for one year from the date of filing the request for a backdated withdrawal. Submit during the semester in which the withdrawal is being requested.
- Program/Graduate School Dismissal Appeal
Students who have been dismissed from the Graduate School may appeal for readmission (see graduate catalog for full policy and appeal procedures). Supporting documentation is required as part of the appeal submission. Students should submit this form after consulting with their program director/advisor to determine their support of the student’s appeal.
- Graduate Student General Petitions
This Graduate Student General Petitions form is the official method for submitting Graduate School policy exception requests (i.e., program time limit extensions, course waivers, enroll in excess hours, etc.). Prior to submitting a petition, all students should consult with their program director/advisor to determine their support of the student’s petition. Students are required to provide supporting documentation or information when submitting their request(s).
Forms for assistantships, residency appeals and tuition differential
- Application for Graduate Assistantship
Required of any student applying for a graduate assistantship. The student should submit the application to the department where the assistantship resides. Submit by April 2 - Submit to Department where assistantship resides.
- Tennessee Residency Appeal
Students who wish to appeal their residence classification.
Graduate students classified as non-residents may appeal this classification by submitting a petition and supporting evidence to the Graduate Admissions Office on or before the last day to register for class for each respective semester. These deadlines also apply to students seeking to pay in-state fees due to their full-time employment in the State of Tennessee or receive the Regional Tuition discount. Decisions on appeals made before the deadline will be effective for that semester. Petitions received after the deadline, if granted, will be effective the following semester.
- Regional Tuition Differential
Used by students to apply for the regional tuition differential rate. A student must reside in Catoosa, Dade, Fannin, Murray, Walker, and Whitfield counties in Georgia and Jackson county in Alabama to be eligible. This form must be submitted for every semester of enrollment in order to maintain regional tuition differential rate.

- Degree Objective Change Form
To change degree program.
- Stop Out Request / Leave of Absence
If, through unusual circumstances, the student cannot work continuously in their program of study, the student may request in writing a one-time stop-out or leave of absence. The request must be made no later than the end of the enrollment period of the semester preceding when the stop-out or leave of absence will be initiated.
- Senior Request to Register for Graduate Credit
Seniors who are within 30 semester hours of completing requirements for a bachelor’s degree at UTC may request permission to register for graduate courses. Student must have an overall GPA of 3.0 or higher and be recommended by their major department head as well as the department head of the requested course(s). Subject to the approval of the Dean of The Graduate School, a student may earn up to nine semester hours of graduate credit prior to completing the bachelor’s degree. Courses taken for graduate credit may not be applied toward an undergraduate degree (unless in association with an approved Joint Undergraduate to Masters Program.) Such approval must be obtained each semester by completing this application form.
- Research Involving Human Subjects
The required form is located on the UTC Institutional Review Board webpage. No research involving human subjects can be initiated before the research has been reviewed and approved.
- Research Involving Animal Subjects
The required form is located on the UTC Institutional Animal Care and use webpage. No research involving animal subjects can be initiated before the research has been reviewed and approved.
- TOEFL - IELTS Exemption
International applicants who wish to apply for an exemption to submitting proof of English language proficiency should submit this form.
- GA Nine Hour Waiver
This form is intended to be used by programs to request an exemption from requiring graduate assistants to enroll in 9 credit hours each semester. N.B. Graduate assistants enrolled in the last semester of their program are required only to enroll in the credit hours needed to complete their program of study.
- Graduate Student Travel Awards
The Graduate School has a limited amount of funds set aside for graduate students enrolled in degree programs to participate in professional development for the academic year. Activities that could be funded include: presenting research at a conference, attending a conference, and/or gathering data for research. Graduate students must apply for these funds at least 3 weeks prior to travel.
