Resources for Field Education

The following are resources used for those inside the Social Work field education program. The forms are linked in the name with a description.

• Agency Confirmation Form

  • Students: please take this form with you on the day of your interview, then turn it in to the UTC Social Work Office and attach a business card of your field supervisor.

• Field Hours Forms: Generalist or Specialized

• Supervision Agenda and Reflection Form

• Request for Placement in Place of Employment

  • Students interested in a work placement must submit this form and required attachments to the Field Education Director. Submitting this form does not guarantee approval of the request. Students must have been in their place of employment for a minimum of six months and must have a BSW or MSW field supervisor other than their employment supervisor. The internship must provide the student with experiences that are distinctly different from the student’s regular job responsibilities.
