Please note: The IRB has updated protocol forms. Instead of separate protocol forms for different types of research (exempt, expedited, anonymous surveys), all IRB applications will be submitted using one form, the Protocol Application Form.

Protected Health Information

For all applications involving the use of protected health information, please read and familiarize yourself with the HIPAA regulations and fill out the applicable forms:

• Form G: Request For Waiver Or Alteration Of Subject Authorization For The Use And Disclosure Of Protected Health Information
• Form H: Research Involving Protected Health Information
• Form I: Investigator's Access Preparatory To Research
• Form J: Research Involving Deceased Individuals
