What is Champions Club?

Take charge of your health with the Champions Club, a fun and rewarding program designed to support healthy aging. Complete a checklist of essential wellness activities—like recommended screenings and exams—and earn a prize for your proactive efforts! It’s a simple, motivating way to stay on top of your health.

How it works

• Each participant receives a personal card to track health activities
• When an activity is completed the provider should initial the card
• The MobileMOC team will check cards each month
• Prizes will be awarded along the way
• Those who complete all 14 activities will be honored at an awards ceremony in April 2026
• A consent form is required to participate
• Most services are offered by the MobileMOC team
• The program is open to any adult age 55 and older
