MobileMOC delivers innovative, interprofessional, and community-centered healthcare to older adults across rural regions. Through experiential learning, collaborative practice, and compassionate outreach, we empower patients, support caregivers, and strengthen community health systems. Our mobile, flexible model promotes healthy aging, enhances accessibility, and trains the next generation of healthcare professionals to deliver holistic, equitable, and person-centered care.

Vision Statement

We empower healthy aging in rural communities by developing a sustainable, data-driven health model that ensures older adults and caregivers receive accessible, person-centered care. Through a skilled interdisciplinary workforce, strong community partnerships, and shared learning, we build resilient, age-friendly communities where older adults can thrive in the places they call home.

Pillar 1: Clinical reach

Expand target population to vulnerable adults 18+ (Year 2)
MobileMOC will broaden access to care by expanding services beyond older adults to include vulnerable adults ages 18 and older. This growth allows us to address gaps in care for underserved populations across Southeast Tennessee.
Implement nurse-led health hubs in 8 rural counties (Year 3)
We will establish nurse-led health hubs to increase access points for care in rural communities. These hubs will provide consistent, community-based services while strengthening local healthcare infrastructure.

Pillar 2: Financial sustainability

Leverage collaborative partnerships for shared resources and services (Year 2)
MobileMOC will expand partnerships with healthcare organizations and community stakeholders to share resources, reduce costs, and increase service capacity. These collaborations will strengthen long-term sustainability.
Target 50% earned revenue. (Year 3)
We aim to build a balanced funding model by increasing earned revenue through billing and services. This will reduce reliance on grant funding and support long-term program stability.

Pillar 3: Workforce and education

Build capacity for additional interprofessional education opportunities (Year 2)
MobileMOC will expand training opportunities for students across disciplines, including nursing, occupational therapy, social work, and public health. This strengthens the future workforce while supporting team-based care.
Launch a rural residency pathway for graduate nursing programs (Year 3)
We will develop a rural residency pathway to prepare nurse practitioners for practice in underserved communities. This initiative supports workforce development and long-term access to care in rural areas.

Pillar 4: Community partnerships

Enhance partnerships with established health councils and rural stakeholders (Year 2)
We will deepen collaboration with local health councils and community leaders to better align services with regional needs. Strong partnerships ensure more coordinated and effective care delivery.
Provide training and support for local community groups (Year 2)
MobileMOC will equip community organizations with tools and training to support Age-Friendly initiatives and address social determinants of health. This helps build local capacity and strengthens community-driven solutions.

Pillar 5: Data and outcomes

Establish a public-facing data dashboard (Year 2)
We will develop a transparent data dashboard to share outcomes, track impact, and inform stakeholders. This supports accountability and continuous improvement.
Demonstrate a sustainable model for scale and replication nationwide (Year 3)
MobileMOC aims to create a proven, scalable model for mobile healthcare delivery. By documenting outcomes and best practices, we will support replication in other regions across the country.
