• TB skin test (scheduled with group)
• Urine drug screen
• Proof of ALL vaccines
• A positive Hepatitis B titer, MMR titer and Varicella titer
• One TDaP (tetanus diphtheria and acellular pertussis) in your lifetime, then a Td booster every 10 years
