Financial Aid Forms
Once your FAFSA is completed, you may be selected for a process called verification by the Department of Education (DOE) and in some cases, by the school. If so, the Financial Aid Office will request prior year income documents and/or household information. You will be sent an e-mail to check your MyMocsNet account for any additional documents requested. Required forms and documents are accessible for viewing through your MyMocsNet account.
Beginning Fall 2012, we can no longer accept a copy of your tax return if selected for verification. We encourage you to document requested tax information using the IRS Data Retrieval Tool. This process allows all required data to match to the FAFSA from the IRS website. We will upload your updated FAFSA within 48 - 72 hours. If you are unable to use the IRS Data Retrieval Tool, you may request a copy of your tax return transcript online in pdf format at www.IRS.gov.
All 2013-2014 documents may be submitted to the Financial Aid Office either by fax: 423-425-2292; by email: firstname.lastname@example.org or in person.
All 2014-2015 documents must be submitted using our online document process.
