ANNUAL ENROLLMENT
The Annual Enrollment Transfer Period for insurance benefits is October 15 through November 14. This is the only time of the year when participants can make changes in their insurance coverage. Changes in coverage will become effective on January 1, with the exception of Unum Provident Optional Life Insurance.
UnumProvident will inform you of the effective date.
If you are not making any changes to your insurance coverage, there is nothing you need to do. All forms must be received in the Office of Human Resources no later than 3 PM on November 14, 2008. No changes/enrollment will be accepted after that time. Faxes cannot be accepted. Please review the benefit changes and premium changes for 2009, listed at the bottom of the page.
If you are not currently participating in a health plan with the University, you cannot enroll UNLESS you apply for medical coverage as a late applicant, for a $60 non-refundable fee, or apply for Special Enrollment by Qualifying Event. Additionally, you cannot add dependents that are not currently covered unless you have a qualifying event.
During annual enrollment, participants can:
- Transfer Health Insurance coverage, if currently enrolled, or cancel
- Enroll in, transfer, or cancel Dental Insurance
- Enroll in, change coverage, or cancel Optional Special Accidental Death & Dismemberment
- Enroll in, increase or decrease current benefit level, or cancel Optional Term and Universal Life
- Enroll in, increase or decrease current benefit level, or cancel Long Term Disability
- Apply for Long Term Care Insurance through MedAmerica Insurance Company
Benefit Changes for 2009
PPO Blue Cross/Blue Shield:
Annual Medical Deductible (In-Network): Increase to $350/individual; $875/family
Annual Medical Out-of-Pocket Maximum (In-Network): Increase to $1,350/individual; $2,700/family
Emergency Room Use: Increase to $75 per visit
Prescription Drug CoPay: Increase to $25/preferred brand; $50/non-preferred brand
Annual Pharmacy Copay Maximum: Increase to $1,620/individual
Diabetic Supplies (strips, lancets, syringes): Coinsurance waived when using In-Network Provider
Diabetic Prescription Drug (oral and insulin): Copay waived for generic and preferred brand when using In-Network Provider; $50 for non-preferred brand
POS Cigna:
Physician Office Visit (In-Network): Increase to $25 copay for PCP; $30 copay for specialist
Maternity (In-Network): Increase to $25 copay; $30 copay for specialist
Chiropractic Care: Increase to $25 copay
Emergency Room Use: Increase to $75 per visit (This applies only if not admitted to the hospital.)
Prescription Drug Copay: Increase to $25/preferred brand; $50/non-preferred brand

HMO United HealthCare:
Physician Office Visit: $20 copay PCP; $25 copay specialist
Maternity: Increase to $20 copay PCP; $25 copay specialist
Chiropractic Care: Increase to $20 copay per visit
Emergency Room Use: Increase to $75 copay per visit (This applies only if not admitted to the hospital.)
Prescription Drug Copay: Increase to $25/preferred brand; $50 non-preferred brand

Delta Dental:
Coverage for Endostal Implants - Included as a Major Service covered at 50% co-insurance level. The maximum allowable cost for an implant under the plan is $1,800. This benefit is included in the annual maximum benefit of $1,000.
Additional cleanings for covered members with certain high risk health conditions. Two additional cleanings for those with specific high risk conditions will be available at the 100% benefit level for Diagnostic and Preventative Services.
