DENTAL INSURANCE
Enrolling, terminating, or changing dental insurance plans may be done only during the Annual Enrollment Transfer Period (October 15 - November 14), with an effective date of January 1st.
Plans you may choose from are the Prepaid Plan administered by Assurant or the Preferred Provider Organization (PPO) administered by Delta Dental. When choosing a dental plan, view a comparison of plans and comparison of premiums.

You must choose a primary care dentist from the network; however, each family member may choose a different dentist. There are some co-pays for dental procedures; some procedures at no charge; no claim forms to submit; pre-existing conditions are covered; no maximum benefit levels; no deductibles; 25% discount on all orthodontic services; no age or dollar maximum restrictions on orthodontics.
There are no changes to benefits or premiums in 2009 for the Prepaid Plan.

- Primary Care Dentist Card must also be submitted if you are enrolling in the Prepaid Plan. Enrollment forms will not be accepted without the Primary Care Dentist Card.

You may select any dentist; no referrals are required and you or your dentist will file claims for covered services. Some services require waiting periods and limitations and exclusions apply. No deductible per individual for in-network provider; $100 deductible per individual for out-of-network provider; benefits for covered services paid at the lesser of dentist charge or the scheduled amount; $1,000 annual maximum benefit per person.
What is the Delta Dental "Safety Net"? If you visit a dentist who is not a Delta Dental PPO provider but is a Delta Dental Premier provider, the amount you may be balance billed is limited. This may be an additional savings to you and your family members.
Benefit Changes for 2009
Coverage for Endostal Implants - Included as a Major Service covered at 50% co-insurance level. The maximum allowable cost for an implant under the plan is $1,800. This benefit is included in the annual maximum benefit of $1,000.
Additional cleanings for covered members with certain high risk health conditions. Two additional cleanings for those with specific high risk conditions will be available at the 100% benefit level for Diagnostic and Preventative Services.
