bullet ENTRY FORM MUST BE POSTMARKED OR HAND DELIVERED NO LATER THAN FRIDAY, FEBRUARY 10, 2012 AT 5:00PM.
bullet Third class mail does not always bear a postmark.  Certified mail is suggested.
bullet Entry forms may be hand delivered to Ginny Reese at UTC no later than 5:00 pm Friday, February 10, 2012.  Entry forms can be delivered to Ginny Reese in 103 Hooper Hall on UTC's campus, between the hours of 8:30 am and 5:00 pm, Monday through Friday.
bullet All Entry forms must be accompanied by an $10.00 entrance fee (payable to Chattanooga Regional Science Fair).  Teams must pay only $10.00 total, not per person.
bullet Entrance fee can only be paid by check or money order if sending entrance form by mail.  If hand delivering form, entrance fee can be paid by cash, check, or money order. Checks must be MADE PAYABLE TO CHATTANOOGA REGIONAL SCIENCE FAIR.
bulletSenior Division students, make sure that all required forms have already been submitted to the Science Fair Office:

  1. Checklist for Adult Sponsor/Safety Assessment Form

  2. Student Checklist (1A), including the Research Plan Attachment

  3. Approval Form (1B)

Senior Division students must also submit additional forms for experiments involving animals, human subjects, tissue research, pathogenic agents, controlled substances, hazardous substances or devices, lasers, recombinant DNA, or Gasohol.  These additional forms must be submitted to the Science Fair Office for approval by the Scientific Review Committee prior to beginning work.  If you are unsure whether or not you need to submit any of these forms, please check with the Wizard on the ISEF website.

All research requiring a Federal and/or State Permit will be expected to have the permit prior to the onset of the research.  It must also be submitted with entry form.
