Referrals
Please be familiar with YOUR insurance. Your insurance plan may need a referral from your Primary Care Physician (PCP) before you can be seen here. If this is the case, then you are responsible for that referral -- it could mean the difference between the insurance company or YOU paying the bill. If your plan requires a Primary Care Physician (PCP) you may want to change your PCP to our providers here. Some of the plans that may need a referral or a change in PCP are Bluecare, Heritage Select/State of TN, any HMO plan or TRICARE PRIME. If you have questions about your insurance coverage, please call your insurance company at the number listed on the back of your insurance card.
If you have Aetna through the University, you can go to any provider who accepts Aetna and NO REFERRAL IS REQUIRED.
If you would like to have an appointment with a specialist, such as a dermatologist or urologist, it is best to call your insurance company first to determine participating providers in this area. We will be glad to assist you in any way we can once you have contacted your insurance company.
Records Requests
Records cannot be sent to other facilities (physician offices) or to you by phone request. You must come to our office and sign a release for your records. -- THIS IS A Law!
If our office refers you to a physician or facility we will take care of sending your records and you do not have to request these.
If you are requesting immunization records please read the following carefully.
Immunization forms are NON-TRANSFERABLE!!! It is your responsibility, as a student in any course of study, to keep copies of your immunization records! Once your form is submitted to the University it will not be returned to you or copied for any reason.
