- Social Security Card (original) AND Drivers License
- OR Passport
- OR Other I-9 verification material

- Bank deposit slip (for direct deposit [required])
Health Benefits
- Beneficiary name, address, social security number, date of birth
- Dependent names, addresses, social security numbers, dates of birth
- Certificate of Insurance from previous employer (if you have been covered by health insurance coverage within 63 days of employment with UTC).
Retirement
- Beneficiary name, address, social security number, date of birth
Parking
- Parking lot you'd like to park in (1st, 2nd, & 3rd choices)
- Year, make, model of car(s) you want to register
- License tag of car(s) you want to register
Voter Registration
- Birth date
- Home address (in Tennessee/Georgia)

You MUST choose ONE plan; benefit is paid totally by the State of Tennessee

May choose ONE plan, 80% of actual premium paid by the State of Tennessee, employee’s 20% portion is pre-tax
