12:00 - 1:00 pm Lunch (Chickamauga Room)
1:00 - 2:30 pm Benefits Information & Retirement (continued)
2:30 - 2:45 pm Faculty Governance, Faculty Handbook, Faculty Committees, Service to the University
2:45 - 3:15 pm Break (Complete scavenger hunt & sign up for individual benefits sessions, if needed)
3:15 - 4:15 pm Diversity & Legal Information
4:15 - 4:30 pm Summary and Wrap up for the Day

Payroll

  • Social Security Card (original) AND Drivers License
  • OR Passport
  • OR Other I-9 verification material

ALSO

  • Bank deposit slip (for direct deposit [required])

Health Benefits

  • Beneficiary name, address, social security number, date of birth
  • Dependent names, addresses, social security numbers, dates of birth
  • Certificate of Insurance from previous employer (if you have been covered by health insurance coverage within 63 days of employment with UTC).

Retirement

  • Beneficiary name, address, social security number, date of birth

Parking

  • Parking lot you'd like to park in (1st, 2nd, & 3rd choices)
  • Year, make, model of car(s) you want to register
  • License tag of car(s) you want to register

Voter Registration

  • Birth date
  • Home address (in Tennessee/Georgia)

You MUST choose ONE plan; benefit is paid totally by the State of Tennessee

Health Benefits (Time Sensitive)
May choose ONE plan, 80% of actual premium paid by the State of Tennessee, employee’s 20% portion is pre-tax
