IRB FORMS
The following forms should used for IRB applications. All Word documents use the form lock feature which means that you can only type in the fields where information is requested. This will show as gray boxes on the form. Simply place your cursor in the gray area and type.
If you need to add additional pages, documents, or appendices to the forms, you will need to "unlock" the documents. In Word 2003, you may accomplish this by clicking Tools/Unprotect document. You may also open the Forms toolbar (View/Toolbars/Forms) and click the padlock icon. Please contact instrb@utc.edu if you have any problems with this.
