Forms for Current Students
Required for All Degree Seeking Students
- Admission to candidacy
When approved, the form serves as a contract for completing degree. Meet with academic advisor to discuss the courses required to complete degree. Submit to the Graduate School after the completion of between 9-18 semester hours of program coursework. Late forms must be accompanied by a Graduate Student Petition Form.
- Graduation application
Submit intent to graduate to the Records Office. Deadlines: g raduating Fall semester, June 15; graduating Spring semester, October 15; graduating Summer semester, March 15.
Other Forms
- 400-level form (for courses that have been approved)
- 400-level form (for courses that have not been approved)
Must be submitted if you are enrolled in a 400-level course for graduate credit. Submit by the last day of the semester to add coursesor the credit will be changed to undergraduate credit by Graduate School staff. While the faculty member must approve the form, it is your responsibility to assure the form is submitted to the Graduate School.
- Committee Appointment-Thesis/Dissertation
- Certificate completion application
Must be submitted if you seek a certificate, rather than a degree. Submission and approval are required prior to posting of certificate completion on your transcript.
- Backdated withdrawal form
Must be submitted if you believe the course drop date should be backdated to a previous date. Reasons for doing so may include medical or other personal issues which prevented you from completing the class and/or withdrawing within the appropriate time period. Backdated withdrawals often affect fee charges. Submit for an indefinite period of time.
- Comprehensive exam
Submit if your program of study requires comprehensive exams. Submit to the Graduate School for approval at least one month prior to the exam date.
- Degree objective change form
Submit if you wish to change your degree program. Other application materials may be required. Full admission into one degree program does not guarantee full admission into another. Non-degree seeking students should complete this form in order to pursue admission to a specific program. Submit at the time you decide to change desired program.
Grade Appeal
Submit if you have reason to challenge an awarded grade. Note specific timelines listed on the form instructions.
Graduate Council Petitions Form
Submit as needed to petition various exceptions to Graduate School policies. Submit in a timely manner as needed. Form must first be reviewed by the program before review by the Graduate Council Petitions Committee.
- Graduate School Tennessee Residency Appeal Form
Submit to the Graduate School if you want your original residency classification to be reviewed. Note specific submission deadlines each semester in the instructions. The form must be notarized to be valid.
- Individual Studies Contract
Submit this form, along with an approved registration form, if you plan to enroll in independent study courses. These course numbers include all 495r, 497r, 498r, 597r, and 598r courses in the various disciplines.
- Information update
Document changes with the Graduate School such as name, address, and phone. Name changes must be accompanied by a legal document. Submit in a timely manner, as needed.

- Revision to candidacy
Revise your program of study, following approval of the initial Application to Candidacy. These changes must be approved by the advisor, Graduate Program Coordinator, and the Director of the Graduate School. Submit to the Graduate School following a meeting between you and your advisor and approval by your graduate program coordinator.
- Graduate Assistantship application
Degree seeking students desiring to apply for a graduate assistantship should complete this form by the priority deadline of February 15. These applications will be considered for the following academic year.
- Opportunity Assistantship application
First time and first generation degree-seeking graduate students may apply. The form should be completed by the priority deadline of February 15 to be considered for the next academic year.
- Graduate Scholarship application
- Transcript request
Complete this form if you need to request that another institution send a transcript to UTC. Send this form to the institution from which you are requesting the transcript.
- Transient letter of good standing
If you are enrolled at another institution and wish to take courses at UTC, you should submit this form to the institution of primary enrollment. The institution should complete the form and submit to UTC.
- UTC senior request for graduate credit
You should submit this form to the Graduate School if you are a UTC senior within 30 semester hours of graduation and wish to take courses for graduate credit.
