Acceptable Tax Documents for Verification
The following tax documents are considered acceptable for verification purposes:
- Form 1040 - 2 pages - must be signed at the bottom of the second page by the student or parent filer. Click this link for an example of this form.
- Form 1040A - 2 pages - must be signed at the bottom of the second page by the student or parent filer. Click this link for an example of this form.
- Form 1040EZ - 1 page - must be signed at the bottom by the student or parent filer. Click this link for an example of this form.
- A copy of your Tax Transcript directly from the IRS - the Tax Transcript is different and more specific than an Account Transcript. This statement may be multiple pages; it must be signed by the student or parent filer on the final page even though there is not a specific signature line.
NOT ACCEPTABLE: Form 8879 - IRS E-file Authorization

The tax document must be signed by one of the filers listed, even if the document has been prepared electronically and has "File Copy-Do Not Sign" printed on the signature line (per federal regulation 34 CFR 668.57).
