Loan Forms
- 2012-2013 UTC Loan Adjustment Request Form - Complete this form if requesting any changes to your loans (offer, declined, decrease or increase)
- 2012-2013 UTC PLUS Information Form - Complete this form once your parent has completed the Parent PLUS Loan application at www.studentloans.gov
- 2012-2013 UTC Private Loan Request Form - Complete this form only if you have applied for a Private Loan with a private lender
Appeal Forms
- Satisfactory Academic Progress Appeal Form: This form is used if you have/had grade/credit hour troubles due to circumstances beyond your control.
- UTC Scholarship Appeal Form: This form is used if you have/had grade/credit hour troubles due to circumstances beyond your control.
- HOPE Scholarship Appeal Form: This form must be submitted within the first two weeks of the semester following the notification of loss of eligibility. Dropping below the minimum hourly requirement is NOT grounds for appeal.
