2007-2008 Academic Year (Fall 2007, Spring 2008, Summer 2008)

Please note: These forms may not be submitted online.  All forms must be printed and faxed, mailed, or brought to the Financial Aid Office.

Verification Forms - Please submit ONLY if requested by the Financial Aid Office

  • Verification Worksheet: This form is to be completed by all students that have been selected for verification.
  • No Taxes Filed Statement: This form is to be completed if either the student, spouse, or parent did not and will not file a 2006 tax return.
  • Minimal Income Statement

A student is classified independent ONLY if one or more of the following applies:

  • the student is 24 years old or older.
  • the student is married.
  • the student has a child or another dependent and provides more than half of their financial support.
  • both of the student’s parents are deceased.
  • the student is an orphan or ward of the court.
  • the student is currently serving on active duty in the U.S. Armed Forces for purposes other than training.
  • the student is a veteran of the U.S. Armed Forces.
  • the student has been admitted to a Master's or Doctoral program.

Other Financial Aid Forms

  • Request for Counselor Review: This form is required for students expecting external scholarships (not UTC or HOPE Scholarships) or other resources, such as a dependent fee waiver or Veteran’s benefits. This form may also be submitted AFTER a student has received a 2007-08 Award Notification if changes are being requested.
  • 2008 Summer PLUS Information Form: This form is required for all parents interested in a Parent PLUS loan for the summer 2008 term.  The parent must be pre-approved by their selected lender before submitting this form.  PLUS loans cannot be processed until this form is received by UTC.
  • Satisfactory Academic Appeal Form - This form is used if you have/had grade/credit hour troubles due to circumstances beyond your control. Faxed appeals will not be accepted.
  • Standard Policies and Procedures: This document is for student and parent informational purposes only and should not be submitted to our office.
  • Treatment of Title IV Aid When a Student Withdraws: This document is for student and parent informational purposes only and should not be submitted to our office.

NOTE: Your MocsNet account will be the primary way the UTC Financial Aid Office communicates with you about your financial aid application, including requests for additional documents. It is important that you check your MocsNet often.
