Setup and Requirements
To view the UTC Co-op online forms, you must first have either Adobe Acrobat or Adobe Acrobat Reader 4.0 or better installed on your computer. Acrobat 3.0 WILL NOT WORK with the UTC Co-op online forms. Acrobat Reader is available for free from adobe.com. When installing, make sure to read and follow the instructions carefully.
All of our forms have been made interactive. You may type directly onto the form and print the form using Acrobat Reader.
To submit a form online, you must be using the browser plug-in version of Acrobat Reader, and either Netscape 4.0 (or better) or MS Internet Explorer 4.0 (or better). If you have the plug-in version, the form will appear in your browser window, and look something like this:

If you do not have the plug-in version, and you would like to submit a form online, simply re-install Acrobat Reader. If this doesn't work, refer to the Help Guide in Acrobat Reader.
Interacting with the Forms
The Acrobat Toolbar should appear right above the form.
To make the form appear
in an appropriate size, select the Zoom Tool:

Next move the pointer onto the form and click until the text is large enough
to read.
Make sure that the Hand Tool is selected. This tool allows you to manipulate (fill in, print, and submit) the form.
Do not use the Text Select Tool. Don't be fooled... You cannot manipulate the form using this tool.

Position the pointer on a form field and click. The I-beam cursor indicates that you can type in a given field; the pointer cursor allows you to select a button, a check box, a radio button, or an item from a list.
Once you click on any form field, you may then tab and shift+tab to navigate between fields.

When you have completed a form, you may print the form by selecting the printer
icon from the Acrobat Toolbar or by clicking on the "Print Form" button
at the bottom of the form. You should print any form that you will need for
your personal records. Some forms must be printed out and delivered to the
Co-op Office.

Some forms may be submitted online. A Submit Form button will appear at the
bottom of these forms. Once you have completed a form, click on the Submit
button. If you have omitted required information, you will be warned before
the form is actually submitted.
IMPORTANT: Unfortunately, you may not save anything other than the blank form using Acrobat Reader. If you are using the full version of Acrobat, however, you may save a filled-out form, or even export the completed form fields to an FDF file.
