UTC OUTSTANDING SERVICE AWARD NOMINATION FORM
Please complete the Nomination Form (Adobe pdf) in full and include supporting documentation. Nominees will not be contacted, therefore it is very important that nominations are accompanied by specific documentation about the nominee's accomplishments and activities.
AWARD CRITERIA
- Outstanding service to the University (recruiting, advocacy, fundraising, faithful commitment, etc.)that rises above the usual standard of service
- Community involvement i.e. civic endeavors, philanthropic work, etc.
- May be given to an individual who is not an alumnus/alumna of UTC
- Award will be given to one recipient per year
- Recipient must accept the award in person unless given posthumously
