UTC DISTINGUISHED ALUMNI AWARD NOMINATION FORM

Please complete the Nomination Form (Adobe pdf) in full and include supporting documentation. Nominees will not be contacted, therefore it is very important that nominations are accompanied by specific documentation about the nominee's accomplishments and activities.

AWARD CRITERIA

• National or international reputation in the alumnus’ field
• Outstanding contribution to science, medicine, the arts, or other fields
• Achievements, patent on a new discovery, or an innovation in a new technology
• Selection for national or international awards such as Pulitzer Prize or Nobel Prize
