Setup and Requirements
To view and print the online PDF forms, you must first have either Adobe Acrobat or Adobe Acrobat Reader, version 4.0 or better, installed on your computer. Acrobat 3.0 WILL NOT WORK with the forms. Acrobat Reader is available for free from Adobe.com's Acrobat pages. When installing, make sure to read and follow the instructions carefully.
