Academic Recommendations
An important component of a professional school application is an academic evaluation from college professors. When you receive a faculty evaluation request from a professional school make an appointment with the appropriate faculty advisor. UTC utilizes a composite preprofessional evaluation form for faculty recommendations to health professional schools. Many schools send their own evaluation form but most accept the composite form. Ask three to five professors who know you well to complete an evaluation form. Make requests for evaluations in person. Select professors in courses where you had a strong academic performance. Most professors will indicate if they do not feel that they can give a good recommendation. It is highly recommended that you supply professors with a summary of your activities including a transcript, work experience, volunteer activities, honors, campus activities, and any other information that will allow the professor to write a thorough recommendation. While it is essential to ask professors from the sciences, evaluations from professors in other disciplines give insight into the breadth of your academic preparation. If you are required to have a character or employment reference contact those individuals and request that they send a letter of recommendation directly to the professional school.
See your preprofessional advisor early in the spring semester of the year you plan to apply and complete the following:

Request for Preprofessional Evaluation. Complete all requested information and indicate when the recommendations are due.

Preprofessional Evaluation Form. Complete the top of each form and sign the waiver of confidentiality. Give one Preprofessional Evaluation Form to each professor who will write a recommendation for you. Ask the professor to return it to your preprofessional advisor. Allow at least four weeks for the faculty member to complete and return the form to your preprofessional advisor.
