Player Registration Packet
You must complete the Registration Form, Initial Change Survey, and Goal Sheet and return them to the HHP Department before you can begin accumulating points. The forms may be returned by hand to the HHP Departmental Office in 205 Maclellan, Via Campus Mail to Chancellor's Challenge HHP #6606 or via email.
